    This Agreement is made and entered into on this day of , by and between
    Hereinafter referred to as the "Participant" AND PJTV23, the local cable television channel, whose current location address is: 20 Hammond St, Port Jervis, NY 12771. Hereinafter referred to as "PJTV23."
    1. Grant of Rights: The Participant hereby irrevocably grants PJTV23, its affiliates, agents, employees, licensees, and assigns the unrestricted right and permission to record, film, photograph, and/or videotape the Participant's likeness, voice, and performance (collectively, the "Footage") during the period of the recording. This includes any interviews, statements, and/or participation in any video content, production, and programming for broadcast or distribution. 2. Use of Footage: The Participant agrees that PJTV23 shall have the right to use, reproduce, distribute, publicly display, broadcast, or otherwise exploit the Footage in any and all media now known or hereafter devised, worldwide, in perpetuity, and without further compensation to the Participant. 3. Waiver of Rights: The Participant waives any right to inspect or approve the finished product, including any use of the Footage in connection with any advertising or promotional materials related to PJTV23's programming. 4. Release from Liability: The Participant hereby releases PJTV23 and its affiliates, agents, employees, and assigns from any and all claims, demands, or liabilities of any kind arising out of or in connection with the use of the Footage as authorized by this Agreement, including any claims for defamation, invasion of privacy, or infringement of any right of publicity. 5. No Obligation to Broadcast: The Participant acknowledges that PJTV23 is under no obligation to use the Footage, broadcast the content, or release the production, and that PJTV23 has the right to modify or not use the Footage at its discretion. 6. No Financial Obligation: The Participant agrees that there is no financial or monetary compensation provided for the use of the Footage, and that the Participant will not receive any further compensation from PJTV23, either now or in the future, for the use of the Footage. 7. Legal Age and Authority: By signing below, the Participant affirms that they are at least 18 years old, or if under 18, that a parent or guardian will sign this form. The Participant affirms that they have the full legal capacity and authority to enter into this Agreement. 8. Governing Law: This Agreement shall be governed by and construed in accordance with the laws of the state of New York.
    9. Signature:
